Canthopexy and Canthoplasty

Eyes are often referred to as the windows to the soul, and for many, a drooping or aging appearance can impact their self-confidence. Canthopexy and canthoplasty are subtle but impactful procedures that are meant to change or support the lateral canthus, the outer corner of the eyelids. These terms are very similar so are sometimes poorly understood and used improperly, even by surgeons.

What is Canthopexy?

Canthopexy is a surgical procedure that supports the outer corners of the eyelids, sometimes elevating them to a higher position. This procedure is often performed on individuals with drooping or sagging eyelids, which can be a result from prior lower eyelid blepharoplasty if skin has been removed. This change in the shape of the eyes can have the undesired result of making them look sad. Canthopexy is typically performed on an outpatient basis and involves making small incisions in the eyelid to tighten and reposition the corner of the eye.

What is Canthoplasty?

Canthoplasty is a surgical procedure that involves the reshaping of the lateral canthus, which is the outer corner of the eye. This is done by repairing and reconfiguring how the upper and lower eyelids meet at the outer corner of the eye. Ideally, this interface is a very sharp angle, producing the aesthetically pleasing “almond-shaped” eye. When prior surgery alters this configuration, the lateral canthal angle is “rounded” which robs the eye of its almond shape, leading to the so-called “Betty Boop” eyes, named for the old cartoon character with a very round eye shape. Canthoplasty often involves incising the lateral corner of the eyelid, releasing or re-configuring scar, i.e. via “z-plasty” or “y-to-v- plasty”, and reforming the angle so it is sharper. This is often done in combination with a canthopexy and is performed to improve the appearance of the eyes. It can be done in conjunction with other eye lift procedures, such as lower lid retraction, or blepharoplasty. Canthoplasty is usually performed on an outpatient basis and involves making small incisions in the eyelid to reshape the corner of the eye.

Benefits of Canthopexy and Canthoplasty

Canthopexy and canthoplasty can restore the natural almond-shaped appearance of the eyelids, widening the eye horizontally, and enhancing the appearance of the eyes. When done in combination with a lower lid retraction, it can have the added benefit of improving dry eye symptoms and vision.

Recovery and Risks

Recovery from canthopexy and canthoplasty typically takes about one to two weeks, with swelling and bruising fading within a week. There is a risk of infection, scarring, and dissatisfaction with the results, as with any surgical procedure. If a patient has had multiple prior surgeries, the success rate of the surgery is diminished.
