This 67-year-old woman presented with complaints of hooded upper eyelids and “bags” underneath her eyes. She actually doesn’t have fat prolapse, but reather has “festoons”. This is the term given to swelling within the skin and underlying tissue below the eyelids, at the junction with the upper cheek area. Complicating things even further is that this patient also has lower eyelid retraction; her lower lids have sagged so that she is showing too much white (scleral show) inferiorly. Dr. Amadi performed lower lid retraction repair with internal graft, lower lid blepharoplasty with skin/muscle flap, festoon excision, and an upper blepharoplasty.
